FourPoints Health continues to work closely with our state and local public health officials to coordinate a comprehensive public health response to the ongoing COVID-19 pandemic.

FourPoints Health continues to work closely with our state and local public health officials to coordinate a comprehensive public health response to the ongoing COVID-19 pandemic.
In addition to the state guidelines, FourPoints Health is taking additional measures to ensure community safety:
- We encourage social distancing, even while in facilities. Upon arrival all patients/clients will be screened for COVID-19 before entering the facility
- All individuals entering the facility must wear a mask at all times (masks may be removed when appropriate for examination or procedures)
- All exam rooms and offices must be sanitized following every in-person appointment/session
- Telehealth will still be utilized for all Group Sessions and as needed for other individual appointments/sessions
All patients/clients will be screened prior to entering the facility. Please review the following:
- Do you have a fever?
- Do you have a cough or shortness of breath?
- Have you had recent contact with someone who has tested positive for COVID-19?
- Have you traveled outside of the United States or to an area where COVID-19 is active in the past two weeks?
- All patients/clients/guests must wear a mask
- Additional cleaning will be performed from clinic staff in between all visits
- Clinic staff will don proper PPE throughout visits
- Proper hygiene will be practiced by all staff members and encouraged for all who enter the facilities
COVID-19 Prevention and Safety
FourPoints Health supports the state of Utah’s ongoing efforts to slow the spread of COVID-19.
The general guidelines remain in effect:
- Maintain 6 feet of physical distance in public settings
- Wear a face covering
- Clean your hands with soap and water for 20 seconds or with an alcohol-based sanitizer
- Cover your mouth and nose when coughing or sneezing. Use a tissue or your upper sleeve, not your hands
- Avoid close contact with anyone who has flu-like symptoms
- If you are feeling sick, please stay home and distance yourself from others
The Utah Department of Health has launched a new information statewide hotline to respond to health-related questions about coronavirus disease. Utahns will be able to call this number and speak with trained healthcare experts with any questions or concerns regarding COVID-19.
