In our efforts to provide an efficient and easy check-in process, FourPoints Health now offers a check-in phone line for all Tele-Behavioral Health appointments. To check in for your Tele-Behavioral Health Appointment, please complete the followings steps:

  1. Complete the required forms below
  2. Ensure you have the proper application installed on your computer or phone
  3. 5-10 minutes prior to your appointment, please call 435-688-8198 to begin the check-in process

Technology Requirements

FourPoints Health proudly offers two Telehealth platforms.  1- Epion Health and 2- Zoom. Epion Health is utilized by our Medical Providers and some Behavioral Health Providers. Zoom is utilized by some Behavioral Health Providers. Please verify with your provider which Telehealth platform you will be using as the requirements for each vary slightly.

  • Epion Health can be accessed using a link sent via text message or email. Please notify the front desk how you would like to receive the link. Standard text message rates apply.
  • The Zoom app can be downloaded for free: https://fourpointshealth.zoom.us/download
